Before filling out a registration form, check you are in our catchment area.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery:
- complete the NHS register with a GP surgery: Stopsley Village Practice online form
Alternatively, you can print the following documents, complete and return them to the practice, bringing with you proof of current address (dated within the last 12 weeks) and photo ID (e.g. photo driving licence or passport).
You can also visit the surgery and collect a registration pack.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
